<br/><br/> RésuméLa parole du psychanalyste avec un patient psychotique peut être vécue comme porteuse d’une dangereuse altérité, ouvrant la voie d’un travail d’errance, voire de souffrance ; elle prend le risque de dire ce que le patient ne peut entendre. À partir de la psychothérapie engagée avec un patient adulte psychotique au sein d’un hôpital de jour, l’article explore trois registres différenciés mais liés de la problématique de ce patient : la dynamique transférentielle, l’environnement familial et l’institution soignante. La référence aux travaux de W. Bion permet d’illustrer les concepts de confusion et d’identification projective.  What the psychoanalyst says when treating a psychotic patient can be experienced as bearing with it a menacing otherness, opening up the way for a travail of mental vagrancy, or even of suffering ; there is the inherent risk of the patient being told what they cannot bear to hear. On the basis of psychotherapy in treating a psychotic adult patient in a day hospital, the article explores three differentiated but related registers of that patient’s problems : the transferential dynamic, the family environment and the healing institution. Reference to the works of W. Bion allows concepts of confusion and projective identification to be illustrated.
