Unique clinical presentation of rabies in a teenager

<br/><br/> Rabies is an almost inevitably fatal disease caused by a neurotropic rhabdovirus. We report the case of an adolescent with pneumothorax, pneumomediastinum and pneumorachis with facial, cervical and thoracic emphysema, severe agitation, acute respiratory distress, cardiac involvement and rash. The history, by the parents, reports a puppy bite a few days before, which was followed by behavioural disorders, including hydrophobia. This case is characterized by cardiac involvement and respiratory distress, rare manifestations of rabies. The respiratory symptoms and emphysema were the result of a rupture of the left lung, due to an effort with a closed glottis produced under the effect of agitation.  La rage est une maladie presque inévitablement mortelle, causée par un rhabdovirus neurotrope. Nous rapportons le cas d’un adolescent présentant un pneumothorax, un pneumomédiastin et un pneumorachis avec emphysème facial, cervical et thoracique, une forte agitation, une détresse respiratoire aiguë, une atteinte cardiaque et une éruption cutanée. L’anamnèse, par les parents, rapporte une morsure de chiot, quelques jours auparavant, à laquelle a succédé des troubles du comportement, notamment une hydrophobie. Ce cas se signale par l’atteinte cardiaque et la détresse respiratoire, manifestations rares de la rage. Les symptômes respiratoires et l’emphysème ont résulté d’une rupture du poumon gauche, due à un effort à glotte fermée produit sous le coup de l’agitation.
