How can we ensure global care for a home death when patients wish this, with medical skills, family support, and input from social work and health care professionals? By thinking of a network as a health care team, which includes different professionals, and has the means to coordinate the continuity of care. In this 3-year experience from Haute-Savoie, nursing services work directly in contact with the patient, physicians, the private practice nurse, and the patient’s family. Medical services act on demand from the primary care physician or from the coordinating nurse. This article presents a quantitative and qualitative evaluation, complemented with family interviews. Globally, the network functions well. Points to strengthen include access to training, collaboration with emergency services, and ease of access to patient files.
