N2  - In patients who underwent surgery for pancreatic cancer and who can start treatment within three months of surgery, adjuvant chemotherapy is currently recommended to limit the risk of disease recurrence, but questions remain for the clinician. Recently, modified FOLFIRINOX has become the standard in the non-Asian population, but there is still a risk of toxicity and its administration may be limited in heavily pretreated patients or those who have not enough recovered from surgery. The gemcitabine-nab-paclitaxel combination, gemcitabine or 5-FU single agents are alternatives to discuss for patients not eligible for FOLFIRINOX. In Asia, chemotherapy based on the compound S1 is the reference. The objective of this review is to summarize the adjuvant management after resection of pancreatic cancer and to highlight current and future questions, in particular the need for biomarkers and the optimization of the management of patients; Chez les patients opérés d’un cancer du pancréas qui peuvent commencer le traitement dans les trois mois suivant la chirurgie, la chimiothérapie adjuvante est actuellement recommandée pour limiter le risque de récidive de la maladie, mais des questions subsistent pour le clinicien. Récemment, le FOLFIRINOX modifié est devenu le standard dans la population non asiatique, mais il existe toujours un risque de toxicité et son administration peut être limitée chez les patients lourdement prétraités ou ceux qui n’ont pas suffisamment récupéré après la chirurgie. L’association gemcitabine-nab-paclitaxel, la gemcitabine ou le 5-FU seuls sont des alternatives à discuter pour les patients non-éligibles au FOLFIRINOX. En Asie, c’est la chimiothérapie à base du composé S1 qui est la référence. L’objectif de cette revue est de résumer la prise en charge adjuvante après résection d’un cancer du pancréas et de mettre en lumière les questionnements actuels et futurs, en particulier le besoin de biomarqueurs et de l’optimisation de la prise en charge des patients
