N2  - RésuméL’immunothérapie par inhibiteurs des points de contrôle immunitaire a profondément modifié la prise en charge des patients atteints de carcinome épidermoïde des voies aérodigestives supérieures en rechute ou métastatique. Cependant, seule une minorité de patients en bénéficient à long terme, alors que la plupart résistent ou progressent après une réponse initiale. De nombreux essais cliniques sont en cours pour améliorer ces résultats, mais aussi pour essayer d’incorporer l’immunothérapie dans les stades plus précoces de la maladie, en association à la radio(chimio)thérapie adjuvante ou exclusive. Dans cet article, nous verrons quels ont été les derniers résultats présentés et quels sont les essais cliniques les plus importants en cours, ainsi que les stratégies en cours d’étude pour essayer d’augmenter les réponses à l’immunothérapie; Immunotherapy with immune checkpoint inhibitors has profoundly changed the management of recurrent or metastatic head and neck squamous cell carcinomas. However, only a minority of patients benefit in the long term, and most patients are either resistant to treatment, or the carcinoma progresses after an initial response. Numerous clinical trials are underway to improve this and to incorporate immunotherapy in the earlier stages of the disease, either with or without radio (chemo) therapy. In this article, we outline the latest results and the most important clinical trials underway, as well as ongoing strategies aiming to enhance the response to immunotherapy
