N2  - Squamous cell carcinomas may complicate chronic neglected wounds especially for the diabetic patient. Bone invasion is uncommon but the prognosis is pejorative and indicates most of the time radical treatment. We report here the case of a 62-year-old diabetic patient, who has been consulting for an active lesion since one year of the left forefoot, which is responsible for a sporadic bloody flow. The biopsy found a squamous cell carcinoma. We recommended surgical treatment by transtibial amputation; Les carcinomes épidermoïdes (CE) peuvent compliquer des plaies chroniques négligées, en particulier chez le patient diabétique. L'envahissement osseux est rare mais est de pronostic péjoratif et nécessite un traitement le plus souvent radical. Nous rapportons ici le cas d'une patiente de 62 ans, diabétique, qui présentait une lésion évolutive, depuis un an, présente sur le bord antérolatéral du pied gauche et responsable d'un écoulement sanglant intermittent. La biopsie mettait en évidence un CE. Nous avons retenu l'indication d'un traitement chirurgical par amputation transtibiale
