<br/><br/> RésuméCe travail a pour but de montrer l’importance de l’objet réel et de l’intersubjectivité dans le travail avec des patients somatisants. Selon l’auteur, la problématique narcissique identitaire est en lien avec une relation primaire défaillante responsable d’un défaut du narcissisme. La défaillance de l’objet et le décrochage perceptif entraînent un rétrécissement de l’espace de jeu psychique et un bouillonnement pulsionnel. Ce dernier entre en résonance avec des traces agonistiques de la prime enfance et a tendance à déborder l’appareil psychique et à favoriser des somatisations. Sur le plan technique, l’auteur préconise la co-construction, le partage de l’affect et un travail de métacommunication.  This paper aims to demonstrate the importance of the real object and intersubjectivity in work with somatising patients. The author considers the problematic of identity narcissism to be connected with a defective primary relationship that causes a lack of narcissism. The failure of the object and the breakdown of the perceptual involve a contraction of the space of psychic play and a ferment of the drives. This latter starts to resonate with agonistic traces from early childhood and has a tendency to overwhelm the psychic apparatus and foster somatisations. On the technical level, the author advocates joint construction, the sharing of affect and a work of meta-communication.  ResumenEste trabajo tiene por objetivo el mostrar la importancia del objeto real y de la intersubjetividad en el trabajo con pacientes somatizantes. De acuerdo con el autor, la problemática narcisista identitaria está vinculada a una relación primaria disfuncional responsable de un fallo de narcisismo. El disfuncionamiento del objeto y el desenganche de lo perceptivo conllevan la reducción del espacio de juego psíquico y la agitación pulsional. Esta última entra en resonancia con las huellas agonísticas de la primera infancia y su tendencia es la de desbordar el aparato psíquico y favorecer las somatizaciones. Desde un punto de vista técnico, el autor preconiza la co-construcción, el compartir el afecto y un trabajo de metacomunicación.  ZusammenfassungDiese Arbeit verfolgt das Ziel, die Bedeutung des realen Objekts und der Intersubjektivität in der Arbeit mit somatisierenden Patienten aufzuzeigen. Dem Autor zufolge steht die narzisstische Identitätsproblematik im Zusammenhang mit einem Versagen der Primärbeziehung, die für den mangelhaften Narzissmus verantwortlich ist. Das Versagen des Objekts und die Abkoppelung der Wahrnehmung führen zu einem Schrumpfen des psychischen Spielraums und einem ungestümen Triebgeschehen. Letzteres gerät in Resonanz mit agonistischen Spuren der frühen Kindheit und tendiert dazu, den psychischen Apparat zu überfordern und Somatisierungen zu begünstigen. Was die technische Ebene betrifft, so beführwortet der Autor die gemeinsame Konstruktion (« Ko-konstruktion »), das Teilen von Affekten und eine Arbeit auf der Metaebene.  RiassuntoQuesto articolo ha per scopo quello di mostrare l’importanza dell’oggetto reale e dell’intersoggettività nel lavoro con i soggetti somatizzanti. Secondo l’autore la problematica narcisistica identitaria è legata ad una relazione primaria carente, responsabile di un difetto del narcisismo. La carenza dell’oggetto e lo staccarsi dal percettivo comportano un restringimento dello spazio di gioco psichico e un’ebollizione pulsionale. Quest’ultima entra in risonanza con delle tracce di agonie della prima infanzia e tende a sopraffare l’apparato psichico e a favorire le somatizzazioni. Sul piano tecnico, l’autore preconizza la co-costruzione, la condivisione dell’affetto e un lavoro di metacomunicazione.
