La présente contribution a pour objectif de montrer en quoi le renforcement et l’adaptation du système de soins aux besoins et attentes des personnes âgées aujourd’hui demande une transformation de la manière d’envisager la démarche éthique. Si l’éthique clinique comme démarche d’analyse de situation clinique reste un outil fondamental de la démarche éthique permettant d’identifier et de comprendre les enjeux éthiques d’une pratique gériatrique à la fois complexe, incertaine et devant articuler adéquatement les attentes des patients, la qualité technique des soins et l’organisation d’une prise en charge complexe, ce domaine de soins manifeste le besoin d’une institutionalisation de la démarche éthique. Pour être pertinente, légitime et efficiente, la démarche éthique doit non seulement s’ouvrir aux dimensions organisationnelles du soin mais pouvoir s’inscrire dans la dynamique institutionnelle.

Chapter 4. How to institutionalise the ethical approachThis paper aims at showing how strengthening and adapting the system of care to the needs and expectations of older people today requires a change in the way to conceive the ethical approach. If clinical ethics remains a fundamental tool of the ethical approach in order to identify and to understand the ethical issues of a geriatric practice both complex and uncertain, as well as to adequately articulate the patient’s expectations, the technical quality of care and the organization of a complex support, this area of care manifests the need for an institutionalization of the ethical approach. To be relevant, legitimate and effective, the ethical approach must not only be opened to the organizational aspects of care but also be able to fit into the institutional dynamics.
