RésuméNous rapportons le cas d’une femme de 83 ans admise aux urgences pour une pneumopathie. La numération effectuée sur l’automate DXH (Coulter® ™ UniCel® DxH) retrouvait une anémie normochrome normocytaire, une numération plaquettaire normale et une formule leucocytaire subnormale. La seule alarme émise par l’automate était la présence d’une basocytose. Un contrôle de la formule par cytométrie en flux et sur frottis a été réalisé. Il montrait l’absence de basocytose mais la présence d’une population lymphocytaire B (ratio T/B < 1 en cytométrie en flux) atypique monomorphe autour de laquelle s’agrégeaient les plaquettes. L’immunophénotypage lymphocytaire a permis de mettre en évidence la présence d’un clone B CD5+.

We report the case of an 83-year-old woman admitted to the accident and emergency unit for pneumopathy. The blood cell count on the DXH automaton (Coulter® ™ UniCel® DxH) found a normochromic and normocytic anemia, anormal platelet count and subnormal leukocyte formula. The only alarm raised by the automaton was the presence of a basocytosis. A control of the leucocyte count by flow cytometry and blood smear has been done. It revealed there was no basocytosis, but showed the presence of an atypical monomorphic lymphocytes B population (T/B < 1 ratio in flow cytometry), around which platelets aggregated. The lymphocytic immunophenotyping allowed us to highlight the presence of a CD5+ B clonal subpopulation.
