RésuméLa mauvaise adhérence thérapeutique, dont le taux est estimé autour de 50 %, limite l’efficacité des antipsychotiques et grève le pronostic du trouble schizophrénique. Parmi les facteurs de risque, le niveau d’insight et la qualité de l’alliance thérapeutique semblent avoir un rôle central. Nous avons développé un programme psycho-éducatif ciblé sur chacune des dimensions de l’insight, de façon à favoriser l’obtention d’une meilleure adhérence et alliance thérapeutique. Soixante-treize patients ont participé au programme avec une moyenne de 5,6 patients inclus par groupe. Le niveau de satisfaction global pour le programme est élevé avec un score estimé de 7,5/10 pour les animateurs, de 8,1/10 pour les psychiatres et de 8,1/10 pour les patients. Ces premiers résultats sont très encourageants au vu de l’intérêt qu’a suscité ce programme.

Development of a targeted, insight-based psycho-educational program for patients with schizophreniaEstimated to be around 50%, poor compliance limits the efficacy of antipsychotic drugs and has a negative impact on the prognosis of schizophrenia. Among factors associated with non compliance insight and quality of the therapeutic relationship appear to play a central role. We developed a psycho-educational program targeting each aspect of insight, with a view to enhancing compliance and achieving a better therapeutic relationship. Seventy three patients took part in the program with a mean of 5.6 patients included per group. Global satisfaction with the program was high, with scores of 7.5/10 attributed by the care givers, 8.1/10 by the psychiatrists and 8.1/10 by patients. These results are very encouraging in view of the interest this program has aroused.

ResumenLa mala observancia terapéutica, cuya tasa se estima sobre el 50 %, limita la eficacia de los antipsicóticos y agrava el pronóstico del trastorno esquizofrénico. Entre los factores de riesgo, el nivel de insight y la calidad de la alianza terapéutica parecen jugar un rol preponderante. Los autores han concebido un programa psico educativo basado en cada una de las dimensiones del insight, de manera a favorecer la obtención de una mejor observancia y alianza terapéuticas. 73 pacientes han participado en el programa con una media de 5,6 pacientes incluidos por grupo. El nivel de satisfacción global del programa es elevado con una cifra estimada a 7,5/10 para los animadores, de 8,1/10 para los psiquiatras y de 8,1/10 para los pacientes. Estos primeros resultados son muy esperanzadores visto el interés que suscita este programa.
