L’évolution du système de santé en France va dans le sens du développement de la prise en charge des patients en réseau de soins.Les pouvoirs publics associent les infirmières aux alternatives à l’hospitalisation pour favoriser l’émergence de telles structures.Cette évolution change les habitudes et les besoins des professionnels. La communication entre soignants est la base de ce fonctionnement.Pour assurer la continuité des soins les soignants libéraux et les soignants hospitaliers ont besoin d’information. Or, cette dernière est très souvent absente ou se révèle non conforme aux attentes et aux besoins des soignants.Les motifs seront analysés et quelques propositions seront données.En recueillant et analysant des fiches de liaison, des constats intéressant sont apparus ce qui laisse entrevoir des possibilités d’évolution au niveau du circuit d’information entre l’hôpital et le secteur extra hospitalier.Des propositions seront exposées pour assurer une plus grande cohérence dans la démarche de la continuité des soins.

The evolution of the French health-care system is based on the development of patient management in the health-care system.The public services associate nurses with alternatives to hospitalization in order to favor the emergence of such facilities.This evolution changes the habits and the needs of professionals. Communication between the nurses is the basis of this function.In order to assure the continuity of health-care, both private and hospital nurses need information. However this is either not the case, or is inadequate to the expectations and needs of the nurses.The motives will be analyzed and some proposals offered.In recording and analyzing liaison records, interesting accounts were revealed which foresees the possibilities of an evolution at the information network level between the hospital and the extra-hospital sector.Some propositions will be put forth in order to assure a greater coherence in the endeavor for health-care continuity.
