520 _aWe present a case of a patient admitted to neurology department for confusion state, due to obstructive hydrocephalus. The cytological analysis of the External Ventricular Bypass (EVB) fluid sample, after leukoconcentration in foetal calf serum, revealed the presence of 25% of large cells with a lymphoma appearance. Lymphocyte immunophenotyping confirmed the infiltration by an aggressive large B-cell lymphoma. With this quick diagnosis, the patient was treated in a very short time and survived. This case highlights the benefit of leukoconcentration with the addition of protein serum (here foetal calf serum) for central nervous system lymphoma diagnosis.
