520			_aIntroduction : Les traumatismes représentent une cause importante de morbi-mortalité chez les enfants en France. Leur recensement et description sont une étape essentielle dans l’élaboration de moyens de prévention adaptés. Dans ce but, un thésaurus spécifique des codes CIM-10 relatifs aux circonstances des traumatismes et accidents de l’enfant a été créé dans le département français de la Loire. L’objectif de cette étude est d’évaluer ce thésaurus en termes de contenu et d’acceptabilité aux urgences pédiatriques du Centre Hospitalier Universitaire de Saint-Étienne (France). Matériel et méthodes : Cette étude descriptive s’est déroulée en deux phases. La première prospective sur trois périodes entre mai et octobre 2014 a comparé les codes des dossiers établis par les médecins des urgences à ceux établis à partir du thésaurus. La deuxième, rétrospective a comparé les codes des dossiers de juillet et août 2014 avant l’instauration du thésaurus à ceux de juillet et août 2015 après instauration. Résultats : La première phase montre que sans le thésaurus, il existe une perte de plus de la moitié de l’information. Pour plus de 90 % des dossiers, les circonstances des traumatismes peuvent être décrites par un code approprié. La deuxième phase montre une progression de 13 % du nombre de codes correspondant aux circonstances des traumatismes avec des informations qui restent partielles. Discussion : Le thésaurus est une aide réelle au codage et répond le plus souvent aux attentes des médecins codeurs. Il mérite d’être diffusé pour des études épidémiologiques d’envergure.
520			_aCircumstances of trauma and accidents in children : a thesaurus-based survey Introduction : Injuries and accidents are major causes of morbidity and mortality in children in France. Identification and description of the mechanisms of accidents are essential to develop adapted prevention methods. For this purpose, a specific thesaurus of ICD-10 codes relating to the circumstances of trauma and accidents in children was created in the French Loire department. The objective of this study was to evaluate the relevance and acceptability of the thesaurus in the pediatric emergency unit of Saint-Etienne university hospital. Material and Methods : This study was conducted in two phases. The first, longitudinal phase was conducted over three periods between May and October 2014 to compare codings by emergency room physicians before using the thesaurus with those defined on the basis of the thesaurus. The second phase retrospectively compared coding in July and August 2014 before introduction of the thesaurus with thesaurus-based coding in July and August 2015. Results : The first phase showed a loss of more than half of the information without the thesaurus. The circumstances of trauma can be described by an appropriate code in more than 90% of cases. The second phase showed a 13% increase in coding of the circumstances of trauma, which nevertheless remains insufficient. Discussion : The thesaurus facilitates coding and generally meets the coding physician’s expectations and should be used in large-scale epidemiological surveys.
