520			_aLes modalités de vécus de changements catastrophiques développés par Bion au sujet de patients psychotiques sont tout autant utiles pour approcher cliniquement les changements catastrophiques découlant de transformations psychiques survenant dans le déroulement habituel de cures d’enfant, d’adolescent, d’adulte, sans pathologie notoirement psychotique, mais aussi lors de circonstances extérieures particulières imposant des arrêts de traitement et/ou changements de thérapeute. Cet article présente l’exemple d’un départ à la retraite d’un psychanalyste exerçant en CMPP, soumis donc lui-même à plusieurs principes de réalité (son obligation de retrait d’activité, conjointe à sa propre impuissance à organiser au mieux sur le plan thérapeutique son remplacement auprès des patients suivis). Sont ainsi exposées certaines réactions d’angoisse à ces changements – côté patients et côté analyste-, et analysé la notion d’angoisse de changement catastrophique comme réactivation d’une irreprésentable angoisse inaugurale. L’évocation de différentes sortes d’arrêt de cures vient souligner la nécessité pour le thérapeute d’être attentif autant à ces événements extérieurs de contraintes qu’aux turbulences psychiques internes qui en découlent. L’article évoque aussi le problème épineux et non résolu des modalités de financement des institutions soignantes, ajoutant une contrainte « réalistique » supplémentaire inapte à moduler de façon adaptée sur le plan clinique, de ces situations difficiles de rupture de traitement.
520			_aThe modes of actual experiences of catastrophic changes described by Bion in the psychotic patients are useful as well to address clinically the catastrophic changes ensuing from psychic transformations occurring during the course of ordinary psychoanalytic treatments with children, adolescent and adult without manifest psychotic pathology, and at the time of particular external circumstances entailing breaks in the treatment and/or changes of therapist. This article sets out the example of an analyst retiring from a CMPP (Psycho-Pedagogic Medical Center), and so subjected to the reality principle (both she was obliged to retired from her previous activity and deprived of the possibility to set up the best way to manage her therapeutic replacement to treat her patients). Some anxious responds to these changes – both from the patient and the analyst – are set out and the notion of catastrophic anxiety is analyzed as a reactivation of an unrepresentable initial anxiety which hasn’t be represented. Evoking different kinds of therapeutic breaks is a way to underline the need the therapist has to be attentive both to the external events imposing their constraint and to the psychic turbulences ensuing from these events. The tricky and unresolved problem of how financing the therapeutic breaks in the therapeutic institutions, in order to avoid an additional realistic constraint preventing an adequate clinical solution to these difficult situations, is evoked.
