520 _aWe report a case of spontaneous hydropneumothorax in a 58-year-old man with metastatic gastric adenocarcinoma and left proximal pulmonary embolism. This association of serious pathologies, which could hamper respiratory and hemodynamic function, was discovered during a CT scan indicated for an assessment of gastric tumor extension in a paucisymptomatic patient, despite the association of these potentially serious pathologies. The mechanism could be explained by the direct passage of air from the alveoli into the pleural cavity, due to a pulmonary infarction.
