520 _aThe Patient Protection and Affordable Care Act (ACA) is the most ambitious health system insurance reform project the United States has undertaken since that of President Lyndon Johnson. The reform aims to modernize the health system by mobilizing substantial federal resources for the large-scale funding of experiments based on ideas that have been known and debated for 25 years: disease management, performance-based payment, prevention and screening programs, and fixed-fee payments. Another aspect of the reform aims to reduce waste, fight abuse and fraud, and rationalize the Medicare system. Why has this law, which brings an end to so many abortive attempts, stirred up such a passionate debate between democrats and republicans? What are the challenges the government will have to face, and will it succeed in implementing the law?
