Summary, etc. The comparison of costs between the public and private for-profit sectors comes up quite often in public debate. However, we must be careful not to generalize, as there are a number of elements—with evidence behind them—that explain the difference in costs. First, we show that the literature has identified fairly well how costs in health care institutions are defined. Second, we identify three reasons for these differing costs: the difference in activity, patient recruitment, and the cost of real estate.
