| Summary, etc. | La Haute Autorité de Santé (HAS) a validé en 2022 une recommandation relative à l’annonce d’un « diagnostic psychiatrique sévère » à un « patient » majeur, insistant sur les précautions à prendre et mettant en exergue la participation active du « patient » à ce processus. Nous analysons quelques aspects paradoxaux de cette recommandation : contraste entre son ambition pour la pratique et la situation réelle de la psychiatrie, essentialisation d’un diagnostic « co-construit », exigence de vérité vis-à-vis du « patient » mais banalisation du potentiel traumatique de l’« annonce » (e.g., dans le document-usager), affirmation du caractère exclusivement clinique d’un tel diagnostic selon un protocole heurtant la rencontre singulière de la personne soignée et de son entourage. Ces paradoxes paraissent inhérents à la situation d’une autorité placée dans la position de soutenir la restauration du pouvoir d’agir, qui associe nécessairement ensemble personnes soignées, aidantes et soignantes. |

| Summary, etc. | In 2022, the French National Authority for Health [Haute Autorité de la Santé] validated a recommendation concerning the announcement of a ‘severe psychiatric diagnosis’ to an adult ‘patient’, insisting on the caution to be taken and emphasizing the active participation of the ‘patient’ in this process. We analyze some of the paradoxical aspects of this recommendation: the contrast between its ambition for practice and the actual situation in French psychiatry, the essentialization of a ‘co-constructed’ diagnosis, the demand for truthfulness with regard to the ‘patient’ but the downplaying of the traumatic potential of the ‘announcement’ (e.g., in the ‘user document’), the assertion of the exclusively clinical nature of such a diagnosis according to a protocol that conflicts with the singular encounter between the person being cared for and his or her entourage. These paradoxes seem inherent to the situation of an authority placed in a position to support empowerment, which necessarily involves together the cared-for, the caregivers and the carers. |
